Eligibility forms
The form must be completed by a health professional (occupational therapist, social worker, physician, nurse, etc.). Allow up to 24 business hours for processing.

Admission criteria
Eligibility follows the policy of Quebec’s Ministère des Transports. The main criteria are:
- Unable to walk 400 m on level ground.
- Unable to climb a 35 cm step with support, or step down without support.
- Unable to make a trip using regular public transit.
- Unable to orient oneself in time or space.
- Unable to control situations or behaviours that could harm one’s own safety or that of others.
- Unable to communicate verbally or by gesture. This criterion alone is not sufficient for admission.
