For an out-of-territory reservation, you must complete the form.

The form must be completed by a healthcare professional (occupational therapist, social worker, doctor, nurse, etc.).

Please allow a maximum of 24 business hours for your request to be processed.

Territories

Inside the territory:

  • Coteau-du-Lac, Les Coteaux, Rivière-Beaudette, Saint-Louis-de-Gonzague, Saint-Polycarpe, Saint-Stanislas-de-Kostka, Saint-Zotique, Salaberry-de-Valleyfield, and St-Etienne de Beauharnois

Outside the territory:

  • Châteauguay, Huntingdon, Vaudreuil-Dorion, Beauharnois, Ormstown, and Île Perrot

Transports Québec Eligibility Policy

Main Criteria

  • Inability to walk 400 m on level ground;

  • Inability to climb a 35 cm step with support or inability to descend one without support;

  • Inability to complete a trip using regular public transit;

  • Inability to orient oneself in time or space;

  • Inability to control situations or behaviours that could be detrimental to one's own safety or that of others;

  • Inability to communicate verbally or with gestures. However, this inability alone cannot be considered for admission purposes.
